Modelo de solicitud de acceso al FIVA

IN BOTH CASES (Models A and B) IT IS ESSENTIAL THAT THE PROTOCOL FOR ADHESION BE SENT TOGETHER WITH THE APPLICATION FORM AND THAT THEY ARE DULY SIGNED AND STAMPED (the protocols are included with the forms)

Two access procedures exist:

  • MEDICAL CENTRES AND EMERGENCY SERVICES SUBSCRIBED TO CAS:

    These centres will have access to the page of the CAS service https://cas.tirea.es, where a new option has been incorporated denominated "CONSULTA FIVA".

    In order to activate the access, and given the fact that users are already registered in the system, they simply need to send an e-mail to ccsprod@consorseguros.es or a fax to 91 339 55 14, enclosing Application form A and indicating the full names of the users that also wish to use the F.I.V.A. (Up to a maximum of 3 users per centre or Emergency Service), as well as the Protocol for joining (included in the form) relating to the centre's adhesion to FIVA, which must be signed and stamped by the centre's representative.

    For these purposes the interlocutors for these centres will be considered to be the same persons as have been designated interlocutors in CAS.

    Confirmation of activation of access will be sent by electronic mail to the address given by the user in the application form.

    These centres will access the service through the internet from the address www.consorseguros.es (clicking on the link to the restricted area denominated "CENTROS SANITARIOS" - "ACCESO AL SISTEMA") and after the software has been installed in the designated user's computer so as to guarantee the security of access.

    It is very important to bear in mind that the installation of the software that allows access for FIVA consultation must be installed in the computer of the person who is going to use the facility, with that same person being responsible for the adequate use of the consultation. It will not be possible to access FIVA consultation from a computer that does not have the software installed.

    In order to do this the centres must request access to the FIVA consultation system by filling in Application form B, which should be sent by e-mail to ccsprod@consorseguros.es or by fax to 91 339 55 14 enclosing the Protocol for joining (included in the form) del relating to the centre's adhesion to FIVA, which must be signed and stamped by the centre's representative.

    This model of application comprises the following data:

    • Data relating to the medical centre.
    • Data relating to the interlocutor: so as to facilitate the communication with the medical centre as well as for sending the FIVA certificates that may be requested in paper format. An interlocutor will be designated.

      The interlocutor can also be a user, to whom a user code and password will be assigned.

    • Data relating to the user: in those cases in which the interlocutor does not also wish to be a user a different person must be designated

    Once the access application has been received an e-mail will be sent to the person designated as user to the address stated in the application: the e-mail will contain the user code, a file that contains the software along with the instructions for installing it and a document for acceptance of liability.

This document must be signed by the user and sent by one of the following routes:

  • Fax: to number 91 339 55 14 for the attention of the "Production Department".
  • By mail addressed to "CONSORCIO DE COMPENSACION DE SEGUROS. DEPARTAMENTO DE PRODUCCION, Paseo de la Castellana, 32 /28046 Madrid.

On receipt of the documentation this body will send the password to the user by e-mail guaranteeing secure access to the system.
